Job Description:

We are seeking Support Coordinators to play a critical role in helping us care for the lives of everyone who travels on Victoria's roads.

The successful candidates will be responsible for managing a portfolio of severely injured clients, aligning the provision of medical, rehabilitation and disability services with their individualised plans to achieve maximal independence and participation in all life areas.

This is a rewarding role that requires resilience, an inquisitive mind, and a passion for making a difference. The ideal candidate will have qualifications and/or experience in a relevant industry, including care coordination, outreach services, social work/case management, mental health, allied health, disability or rehabilitation.

The team is highly supportive, fun, caring and engaging, with a focus on delivering life-critical services where it matters.

Key Responsibilities:

  • Managing client portfolios and developing plans in relation to a biopsychosocial model.
  • Coordinating multiple stakeholders and medical, rehabilitation and disability organisations to ensure optimal outcomes.
  • Communicating effectively with clients, employers and treating practitioners to achieve maximum independence and participation in all life areas.
  • Recognising and managing sensitive issues concerning clients and their families.

The successful candidate will be part of our Independence Branch, working closely with a small team of Active Support Coordinators who are passionate about ensuring the best results for TAC clients.

About the Role:

This role at times will require travel for face-to-face visits with clients and potentially their treating teams, therefore you will need to hold a current Victorian driver's licence.

Specialty Areas:

  • The Intensive Support Coordinator manages a portfolio of severely injured clients, on a short to medium term basis, who are currently experiencing highly complex individual situations.
  • The Early Support Coordinator proactively manages a portfolio of severely injured clients in the early stages of their claim, to achieve optimal outcomes.
  • The Active Support Coordinator will manage clients at varying stages of their rehabilitation in the community setting, aligning the provision of medical, rehabilitation and disability services with the client's individualised plan to achieve maximal independence and participation in all life areas.

What You'll Need to Succeed:

  • Qualifications and/or experience in a relevant industry, including care coordination, outreach services, social work/case management, mental health, allied health, disability or rehabilitation.
  • Sound knowledge of disability support services.
  • Resilience, an inquisitive mind, and a passion for making a difference.
  • Understanding of medical, rehabilitation and disability principles and protocols, and the physical and psychological effects of acquired injuries.
  • Ability to recognise and manage sensitive issues concerning clients and their families.
  • Great communicator and can tailor communication style to suit different audiences.
  • Proficiency using motivational interview techniques.
