This is an exciting opportunity for an experienced nurse to work with our Practice Governance team as a Clinical Nurse Consultant on a permanent full-time basis. This role will help inform future decision-making to effectively resource evidence-based, health-related practice governance approaches to achieve optimal outcomes for the range of client needs being supported in a Supported Independent Living (SILS) environment.
Key Responsibilities- Support the Senior Nurse Consultant with reviews, including liaison with operational staff to consolidate any gaps in client health and disability-related supports.
- Identify and escalate any immediate health and disability-related client risks to the Practice Manager.
- Work collaboratively with Operational Staff to implement and evaluate corrective actions and remedies in response to identified immediate client safety risks.
- Provide recommendations to address any gaps or risks for individual clients throughout the duration of the review.
- Function in more complex situations and provide any immediate triage direction and guidance to senior operational staff, where required.
- Contribute to and provide recommendations to the Senior Nurse Consultant to develop a de-identified profile of the range of health and disability needs, complexity, risks across the SILS environment.
- Contribute to and support the Senior Nurse Consultant and Practice Manager in making recommendations to inform future health liaison functions and roles that will effectively support a range of client needs and complexities, including practical resourcing for non-clinical staff to ensure optimal, sustained outcomes for clients.
- Assist and support the Senior RN Consultant to undertake a review of the evidence-base and efficacy of current health-related CCS policies, procedures, tools, and their application by non-clinical staff (at all levels), against best practice.
- Apply a quality framework to improve service delivery and provide recommendations for continuous improvement for clients within the SILS environment.
- Provide health liaison support and resourcing to operational (non-clinical) staff where required by the Senior RN Consultant.
- Actively promote and support a robust and open practice governance culture throughout the duration of the project.
- Collaborate with internal key stakeholders to support the effective outcomes of the project and to enhance outcomes for clients.
- Liaise with clients, families, carers, guardians where required by the RN Consultant alongside the reviews.
- Proven experience within a clinical or practice setting within the Health, Ageing, Disability, or Community Sector.
- Demonstrated capacity to scrutinise and interpret client records, plans, and documentation and undertake relevant auditing, gap analysis, and provide findings and recommendations – individually and systemically.
- Experience in a health liaison role and effectively supporting, resourcing, and training non-clinical staff.
- Strong analytical and conceptual skills within a systems thinking/process-driven approach to solving complex issues.
- Prior knowledge of legislative and regulatory environments, including compliance and quality standards, preferably within a community and/or health setting.
- Proven record in using skills across a complex organisation, developing professional relationships with stakeholders at various levels of the organisation, and managing and influencing through a strong evidence-base.
- Excellent stakeholder engagement skills.
- Bachelor Degree in Nursing (RN) and 3+ years of relevant clinical experience.
- Current driver's license.
- Blue/Yellow Cards.
