As a key member of our healthcare team, this Hospital in the Home role offers an exciting opportunity to deliver high-quality nursing care to patients in their own communities.
The successful candidate will be responsible for assessing patient needs, developing and implementing personalized care plans, and working closely with multidisciplinary teams to ensure seamless service delivery.
Required Skills and Qualifications- Current registration as a Registered Nurse with the relevant state or territory nursing authority;
- Demonstrated expertise in community nursing practice, including wound management, medication administration, and vital sign assessment;
- Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and other healthcare professionals;
