Services to be provided could include wound care, IDC or stoma care, personal care, medication administration and supervision, blood glucose monitoring and application of compression stockings.
Key Responsibilities:
• Provide high-quality, patient-centered care to clients in their homes
• Conduct thorough assessments and develop individualized care plans
• Collaborate with multidisciplinary teams to ensure seamless care transitions
• Maintain accurate and up-to-date records of client care
• Participate in ongoing professional development to stay current with best practices
Requirements:
• Registered Nurse grade 2 with the Australian Health Practitioner Regulation Agency (AHPRA)
• Minimum 5 years of community nursing experience
• Strong initiative and decision-making skills
• Effective time management and problem-solving abilities
• Current driver's license and registered vehicle
• Current NDIS Clearance
