Transform Lives as a Relationship-Driven Case Manager

We're on the lookout for a dedicated individual to join our Youth Services team. This role empowers young people aged 15-25 to take control of their futures, build skills, and make their own path.

The Role:

  • Support young people through case management - listening, guiding, and advocating.
  • Help them set goals, access support, and stay connected with their community.
  • Collaborate with stakeholders to drive client-centered outcomes.
  • Provide ongoing assessment of client issues, safety, needs, and circumstances from initial intake to exit.
  • Develop individual case plans with clients, including interventions that address their needs.
  • Assist clients to access support services and establish community supports, including referrals to specialised agencies.

Requirements:

  • Qualification in social work, welfare, community development, or related field.
  • Demonstrated sector knowledge and skills through previous roles, service, and study.
  • A valid Employee Working with Children Check.
  • Psychological Assessment (Children and Young People Safety Act).
  • Through Their Eyes (Reporting Abuse and Neglect) Training.
  • First Aid Certificate.
