About the Role

We are seeking a skilled Service Delivery Coordinator to join our team in Rockhampton, Queensland. As a key member of our staff, you will be responsible for overseeing the delivery of high-quality support services to our clients.

Job Description
  • Lead and support teams to ensure exceptional care for NDIS participants
  • Supervise support workers and their rosters to maintain efficient operations
  • Mentor and train staff to enhance their skills and knowledge
  • Conduct risk assessments and update client files to ensure compliance
  • Oversee budgets and manage data accuracy to inform strategic decisions
  • Facilitate client intake and individual care planning to meet unique needs
  • Assist with recruitment, onboarding, and professional development to build a strong team
Requirements
  • Minimum two years of experience in the disability sector as a Case Manager or Service Coordinator
  • First Aid/CPR certification, NDIS Worker Screening Card, Blue Card, and valid Queensland driver's license
  • Strong communication and negotiation skills, with excellent time management and problem-solving abilities
  • Proficiency in care databases and Microsoft 365, with experience in community networking and client care planning
Benefits

You will receive a competitive salary of $65,000 - $80,000 per annum, plus superannuation and additional benefits, including relocation assistance, a shared work-based vehicle, comprehensive induction program, potential for career progression, and a retention allowance may also be available subject to continuing employment and a successful annual performance appraisal.
