We are seeking a seasoned Clinical Nurse Manager to lead our team of healthcare professionals in providing high-quality patient care in the comfort of their own homes.

About This Role

This is an exceptional opportunity for a skilled nurse leader to take charge of our clinical operations, ensure seamless delivery of care services, and drive positive outcomes for patients. As a Clinical Nurse Manager, you will be responsible for managing a dynamic team, ensuring compliance with regulatory requirements, and coordinating care planning to meet individual patient needs.

Main Responsibilities

  • Review and refine care plans to guarantee accuracy and compassion
  • Maintain open communication with physicians, patients, and families to deliver personalized care
  • Monitor and improve patient outcomes through data-driven insights and quality improvement initiatives
  • Collaborate with cross-functional teams to achieve organizational goals and objectives
Requirements

To succeed in this challenging and rewarding role, you must possess:

  • A valid RN license
  • At least 3 years of home health experience, with a minimum of 1 year in a leadership position
  • Proficiency in OASIS, EMR platforms, and documentation systems
  • Exceptional leadership skills, with a passion for empowering teams and driving results
  • Excellent communication and interpersonal skills, with a customer-centric approach
