This Care Coordinator Physiotherapist role presents a unique chance to work in a hybrid care coordination and physiotherapy model, providing comprehensive assessments, interventions, and management of clients with diverse health and social needs.

About the Role

  • The successful candidate will oversee a small caseload of clients in the community and within the Emergency Department, conducting holistic assessments, coordinating health and social care requirements, and monitoring client goal achievement.
  • As a Care Coordinator Physiotherapist, you will provide assessment, intervention, and management to achieve optimal functional recovery, encourage self-management, and promote empowerment for clients to enhance their health outcomes, wellbeing, and quality of life.
  • In the ED, you will conduct comprehensive assessments of patients to ensure safe discharges, referrals to appropriate services, identification of risks, and allocation of hospital and community resources in an efficient and cost-effective manner.

Key Responsibilities

  • A minimum of 2 years postgraduate physiotherapy experience in acute, sub-acute, or community settings.
  • Experience with prioritizing patient demands and optimizing resource utilization.
  • Broad physiotherapy assessment and treatment skills, including musculoskeletal, cardiorespiratory, neurological, and geriatric presentations.
