HammondCare is a leading provider of residential aged care and community services. We are seeking an experienced Dementia Care Specialist to join our team as a Clinical Advisor.

Salary: $90,000 - $120,000 per annum

About the Role:

We are looking for a passionate and dedicated professional to provide expert advisory services in dementia care to our staff at all levels. The successful candidate will have expertise in supporting people living with dementia and demonstrated skills in teaching, mentoring, and coaching others.

The role involves providing support to staff to learn, develop, and implement best practice in dementia care by embedding HammondCare's Model of Care and Case Management Framework. As a Clinical Advisor, you will engage in projects and contribute to ongoing knowledge generation and communication in relation to best practice dementia care and support.

Main Responsibilities:

  • Provide dementia-specific expert advisory services within HammondCare services through participation in regular clinical review meetings and on-the-ground support across all cottages for the site allocated.
  • Build staff capacity through communication, education, mentoring, and coaching in practice; this can include spending time with the team on the ground, conducting case study discussions, and reviewing documentations.
  • Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
  • Work on projects and assignments as required and in line with areas of skill, expertise, and knowledge.
  • Participate, contribute, and learn as part of the Dementia Centre team in team meetings, professional supervision, and team activities.
  • Utilise agreed HammondCare and Dementia Centre processes to communicate, monitor, and manage work delegations and projects including evaluation and reporting.

What We Need from You:

  • Expertise in supporting people living with dementia.
  • Demonstrated skills in teaching, mentoring, and coaching others.
  • Experience in Australian residential aged care and/or community aged care.
  • High level of negotiation, communication, and relationship-building skills.
  • Excellent problem-solving and decision-making skills.
  • Ability to prioritise multiple tasks and meet deadlines.
  • Excellent written and verbal communication skills.
  • Proficient in Microsoft Office Suite with excellent administration skills.
  • A current Australian driver's license and willingness to travel.
  • Empathy and patience.
  • Relevant qualifications in Allied Health, Nursing, Aged Care, or Mental Health highly desirable.
