About the Role

We are seeking a dedicated and compassionate Registered Nurse or Registered Midwife to join our team as a Nurse Home Visitor for the Australian Family Partnership Program (AFPP).

Key Responsibilities
  • Deliver the Australian Family Partnership Program (AFPP) to first-time mothers in a home setting, focusing on early emotional and physical health intervention to improve the lives of vulnerable mothers pregnant with their first Aboriginal or Torres Strait Islander child.
  • Work alongside a Family Partnership Worker to deliver program content, building therapeutic relationships, stakeholder engagement, and health promotion.
  • Receive intensive training in AFPP Home Visit Guidelines, screening tools, and other materials to support parents in improving pregnancy outcomes, child health and development, and supporting parents to develop a vision for their own futures.
  • Apply a strength-based approach, focusing on 5 client-centred principles: focus on strengths, focus on solutions, only a small change is necessary, you are the expert of your life, and interest in Aboriginal and Torres Strait Islander Health, midwifery, child health or family-centred practice.
  • Report to and regularly engage in reflective supervision with the AFPP Nurse Supervisor and function as an integral part of the AFPP Home Visiting Team.

Requirements
