Community Health Support Specialist

We are seeking an experienced Community Services Coordinator to lead the coordination and delivery of home support programs in Port Lincoln and Lower Eyre.
Key Responsibilities- Ensure services are culturally inclusive, meet client needs, and empower people to maintain their independence at home.
- Lead a team that works together to deliver vital community and health services to people in need.
- Strong communication, problem-solving, and conflict resolution abilities.
- Proven background in home and community care, including experience with brokerage models, program development, and service delivery.
- Understanding of primary health care principles, the needs of ageing populations and people with disabilities, and relevant legislation.
- Make a real difference and feel like an important part of the community. The estimated annual salary for this position is between $76,561 - $80,256, depending on experience and qualifications. Additionally, we offer a range of benefits, including salary packaging options and relocation assistance.
