
AOD Community Support Specialist

The AOD Care Navigator will be responsible for delivering specialist, culturally responsive treatment and support to clients across the New England region.
Key Responsibilities:- Provide person-led, evidence-based care, including assessments, care planning, case management, and coordination with service providers.
- Deliver support in person, by phone, and online, ensuring clients are linked to relevant health, community, and support services.
- Contribute to group programs, maintain accurate records, and participate in care coordination meetings.
- Regular travel within the region and flexibility to work from designated offices.
- First Nations Staff Network: Annual conference for Aboriginal and Torres Strait Islander staff to connect and embrace culture.
