Job Role:

Transition Care Specialist

We are seeking a Transition Care Specialist to join our team. In this role, you will play a critical part in ensuring the continuity of care for patients in public correctional centres with Drug and Alcohol problems.

Key Responsibilities:
  • Liaise with patients, correctional and community environments to link them into services that cater to their individual needs.
  • Collaborate with other professionals to facilitate seamless transitions for patients upon release to the community.
Requirements:
  • Proven experience in managing continuity of care for clients with complex health needs.
  • Understanding of Drug and Alcohol treatment approaches and working within community-based networks.
  • A recognised allied health discipline qualification.
