About the RoleThis part-time, Wednesday and Thursday role offers the opportunity to provide clinical leadership and mentoring to a team of RNs, ENs, and PCA's. As a Clinical Care Coordinator, you will drive clinical outcomes to maintain high-quality care and be a role model of clinical leadership.
Key Responsibilities- Provide clinical leadership and mentoring to the care team
- Drive clinical outcomes to maintain high-quality care
- Be a role model of clinical leadership
- At least 2+ years of experience in a Clinical Care Coordinator or similar role in the aged care or acute sector
- RN qualification with current unrestricted AHPRA registration
- Excellent and up to date clinical knowledge and assessment skills
- Experience in staff supervision and development
- Strong communication and negotiation skills
- A working knowledge of funding and accreditation processes
- Knowledge of Care Planning
