Job Title: Disability Support Worker

We are seeking a skilled and compassionate Disability Support Worker to join our team at Careerone Partner Network. As a Disability Support Worker, you will play a vital role in providing comprehensive support services to individuals with disabilities.

Key Responsibilities:
  • Clinical Assessment and Support: Conduct thorough assessments of clients' needs, including functional capacity, physical and mental health, and social and emotional well-being.
  • Team Collaboration: Work closely with Client Managers, Registered Nurses, and Documentation Support Specialists to contribute to support plan changes and service improvements.
  • Client Management Support: Support a caseload of clients, ensuring their needs are met and services are delivered effectively.
  • Quality and Compliance: Participate in quality assurance activities and contribute to the ongoing improvement of services.
Qualifications and Experience:
  • Relevant Qualifications: Hold relevant local or overseas qualifications in a disability or health field, particularly in a nursing setting.
  • Clinical Experience: Demonstrate clinical experience in working with people with complex clinical conditions and disabilities.
  • Problem-Solving Skills: Possess evidenced problem-solving skills and the ability to work independently and as part of a team.
  • Communication Skills: Exhibit strong interpersonal and communication skills, including the ability to work with clients, families, and internal team members.
