Key responsibilities include:

  • Providing health assessments and developing care plans that cater to the unique needs of clients

  • Liaising with external health providers to ensure seamless care coordination

To be successful in this role, you'll need:

  • AHPRA registration with at least 2 years' post-qualification experience
  • A strong clinical background with experience in complex care planning, preferably in a community setting
  • Proven experience in both disability and aged care
  • Excellent communication and interpersonal skills
  • Strong problem-solving and time management abilities
  • A current driver's license
  • NDIS clearance (prior to commencement)
  • A willingness to travel around your region as this role is not solely office based
  • Experience providing group training sessions
