Job Summary

We are seeking a skilled Care Manager to oversee the provision of high-quality, person-centered care for our residents.

About the Role

In this key position, you will be responsible for approximately 90 residents in our facility. You will coordinate and oversee the documentation for consumer care, including developing and implementing timelines to meet clinical practice and service requirements. This role requires a demonstrated commitment to continuous improvement, monitoring trends and best practices, and ensuring regular audits and implementation of improvement initiatives.

Responsibilities
  • Coordinate and oversee the provision of high-quality, person-centered care for each resident
  • Develop and implement timelines to meet clinical practice and service requirements
  • Maintain accurate and up-to-date documentation for consumer care
  • Monitor trends and best practices, and ensure regular audits and implementation of improvement initiatives
