- Conducting thorough assessments to identify individual needs and develop tailored plans to enhance quality of life.
- Collaborating with multidisciplinary teams to ensure seamless delivery of care and services.
- Maintaining accurate records and contributing to quality improvement initiatives.
- Eligibility for membership of the Australian Association of Social Workers.
- Bachelor of Social Work or recognised equivalent qualification.
- Proven experience in an acute hospital setting, preferably with a focus on social work.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
