Disease Management Nurse Specialist

We are seeking a skilled Disease Management Nurse to provide care coordination and support to individuals with chronic diseases. This role involves working closely with healthcare professionals, patients, and their carers to develop and implement effective care plans.
Key responsibilities include:
- Assessing patient needs and developing individualized care plans
- Collaborating with multidisciplinary teams to ensure coordinated care
- Providing education and support to patients and their families
- Maintaining accurate records and reporting on patient outcomes
The successful candidate will have strong communication and interpersonal skills, as well as the ability to work autonomously and as part of a team. A degree in nursing and relevant experience in disease management or a related field are required.
