Complex Needs Coordinator

The role of the Complex Needs Coordinator involves providing strategic support and coordination services for individuals with complex requirements. This position serves as a key point of contact for service providers, clinicians, and stakeholders seeking information about support responses.

Main Responsibilities:
  • Deliver consultation and coordination services for clients with complex needs, focusing on Multiple and Complex Needs Initiative (MACNI) and Support for High Risk Tenancies (SfHRT) program initiatives.
  • Establish relationships between departmental program areas and external services, promoting capacity building in support provision for clients with complex needs.
  • Collaborate with service providers to develop integrated responses and promote best practice excellence.
  • Identify system gaps, risk factors, and training opportunities that enhance service systems for clients with complex needs.
Accountabilities:
  • Provide authoritative and expert advice on program-related matters, aligned with organisational goals and objectives.
  • Work closely with the relevant Manager to manage resource allocation and outputs required to achieve service delivery goals and outcomes.
  • Interpret and apply business plans and policies within the area of responsibility, providing guidance on design and implementation.
