About the Role

As a Disability Support Practitioner, you will play a vital role in empowering individuals with disabilities to live meaningful lives. Your primary responsibility will be to deliver personalized support in a group home setting, fostering independence and autonomy.

The ideal candidate will possess excellent communication skills, empathy, and a passion for supporting people with disabilities. You will work collaboratively with clients to develop and implement individualized plans, ensuring their needs are met and exceeded.

Key Responsibilities
  • Providing support with daily living activities, such as personal care, meal preparation, and medication administration
  • Assisting clients with transportation, including using CPA's minivan
  • Encouraging participation in community activities and promoting social inclusion
  • Supporting clients to make informed decisions and build skills for increased independence
Requirements
  • A current NSW Driver's Licence (P2 minimum)
  • Availability for a minimum of 3 days per week, with flexibility to accommodate various shifts
