About the Role
As a Positive Ageing Officer, you will play a vital role in connecting older adults in the community to essential aged care services. Your expertise will be instrumental in providing service navigation support, care coordination, and advocacy to ensure the best possible outcomes for our clients.

Key Responsibilities:
  • Deliver the Care Finder program in the Northern Region, working collaboratively with consortium partners to provide specialist and intensive assistance to older adults.
  • Support clients in navigating the My Aged Care service system, facilitating screening and eligibility for aged care services, and referring for assessment as needed.
  • Conduct high-level check-ins with clients, providing follow-up support once services have commenced, and maintaining client records and data for monitoring, reporting, and evaluation purposes.
  • Network with Care Finder partnering Councils and external agencies to ensure seamless service delivery and coordination.
  • Provide care coordination and navigation support to Carers eligible for the Support for Carers program, ensuring they receive the necessary resources and support.

Requirements
To be successful in this role, you will require:
  • A tertiary qualification in an allied health or relevant profession (e.g., social work, health/community services, or social sciences) and/or substantial experience working with diverse older adults or individuals with complex support needs.
  • Demonstrated knowledge of and experience in strengths-based approaches, providing information, decision-support, linking, advocacy, and casework for older adults with vulnerable profiles and complex needs.
  • A sound working knowledge and ability to navigate the My Aged Care service system.
  • An understanding of Aged Care reforms and their impact on the aged care service system in Victoria.
  • An understanding of Age-Friendly principles and social determinants of health in older age.
