About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Kempsey District Hospital. As a Clinical Pharmacist, you will play a critical role in providing comprehensive clinical pharmacy services and supporting the quality use of medicines.

Key Responsibilities
  • Conduct medication chart reviews and medication reconciliation to ensure accurate and safe medication use.
  • Dispense and supply medication and pharmacy products to patients and healthcare professionals.
  • Coordinate patient medication requirements for inpatient stay, day infusions, and discharge to ensure seamless care.
  • Develop and maintain professional relationships with key stakeholders to deliver exceptional customer service.
  • Support patients, carers, hospital staff, and associated employees to promote health and well-being.
  • Educate and advise staff and customers on health promotion, disease prevention, drug selection, and the quality use of medicines.
