This role is for a dedicated and experienced Clinical Nurse to join our Ambulatory Care Coordination program. The successful candidate will support the clinical team in achieving service goals and providing exceptional care to patients and their families.
The key responsibilities of this position include leadership, expert nursing knowledge, and facilitating care coordination, assessment, advice, and support to patients and their families. You will be responsible for engaging with patients and their families in a clinical setting, acting as an advocate and collaborating with the healthcare team to improve health outcomes and patient satisfaction.
About Our Service
Our Ambulatory Care Coordination program provides out-of-hospital care to patients who may find it difficult to attend hospital appointments or prefer to stay closer to home. We offer primary care services, including assessment, treatment, and management of patients' medical conditions.
Key Skills and Qualifications
- Advanced level of skill in clinical Paediatric or Child Health care
- Demonstrated knowledge and understanding of family structure, culture, and custom
- Proven ability to function effectively in a multidisciplinary team
