Job Overview

This role is for a dedicated and experienced Clinical Nurse to join our Ambulatory Care Coordination program. The successful candidate will support the clinical team in achieving service goals and providing exceptional care to patients and their families.

About Our Service

Our Ambulatory Care Coordination program provides out-of-hospital care to patients who may find it difficult to attend hospital appointments or prefer to stay closer to home. We offer primary care services, including assessment, treatment, and management of patients' medical conditions.
