We are seeking an experienced Enrolled Nurse to join our team as a Community Care Specialist. This is a permanent, full-time role based out of our Osborne Park Service Centre, covering our central suburbs and the Hills.
Key Responsibilities- Provide high-quality care to clients in their homes, working under the clinical direction of a Registered Nurse.
- Assess, plan, and implement individualized care plans to meet the unique needs of each client.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
- Conduct wound care and catheter management, as well as provide medication administration and management.
- Develop and maintain strong relationships with clients, families, and caregivers to ensure optimal care outcomes.
- Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- 2+ years of post-graduate experience in an Enrolled Nurse role, with a focus on community care.
- Medication competency, with evidence of HLTEN507 A/B/C.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- A current driver's license and a reliable vehicle for travel to client homes.
