
Mental Health Support Worker Role

Key Responsibilities:- Conduct thorough mental health assessments and create tailored care plans that meet individual needs.
- Provide emotional support and guidance to service users experiencing various mental health conditions.
- Collaborate with multidisciplinary teams, including NHS staff, private healthcare providers, social services, and voluntary organizations.
- Ensure safeguarding and risk management measures are in place for vulnerable individuals.
- Support hospital discharge planning, crisis intervention, and community reintegration efforts.
- Maintain accurate and up-to-date case records, adhering to relevant legislation and regulatory requirements.
- Degree in Social Work or equivalent qualification.
- Registration with Social Work England or relevant governing body.
- Experience in at least one area: NHS, private hospitals, councils, or community-based mental health services.
- Familiarity with relevant legislation, such as the Mental Health Act, Care Act, and Mental Capacity Act.
- Ability to work independently and as part of a collaborative team.
