About the Role:

  • Conduct client assessments to identify care needs and develop personalized care plans
  • Deliver exceptional home care services to clients, ensuring their physical, emotional, and social needs are met
  • Collaborate with other healthcare professionals to ensure comprehensive care and support
  • Monitor client progress and adjust care plans as needed
  • Provide education and support to clients and their families on health and wellness topics
  • Maintain accurate and up-to-date records of client information and care provided

Requirements:

  • Enrolled Nurse (EN) qualification with current registration
  • Minimum 1-2 years' experience in community nursing
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong time management and organizational skills

Preferred Skills:

  • Experience working in aged care or disability services
  • Knowledge of consumer-directed care principles and Aged Care and Disability Standards
  • Current First Aid and CPR certification
  • Valid driver's license and access to a reliable vehicle

    • Prepare and submit reports as required by regulatory authorities and management

Qualifications and Requirements

  • Qualifications:
