We are seeking a skilled and compassionate Support Worker to join our team at Chorus. As a Support Worker, you will play a vital role in supporting seniors and NDIS customers to remain in their homes, promoting their independence and well-being.
Key Responsibilities:
- Provide tailored person-centred services and activities to meet the unique needs of our customers
- Work collaboratively with customers, their families, carers, and other professionals to ensure seamless support
- Develop and maintain strong relationships with customers, built on trust, respect, and open communication
- Contribute to a positive and inclusive team environment, promoting a culture of respect and empathy
Requirements
To be successful in this role, you will need:
- Certificate III Individual Support - Aged Care or Disability, or other relevant qualification and/or experience working with seniors, people on the mental health recovery journey, and those living with disability
- Excellent communication and interpersonal skills, with the ability to build strong relationships with customers and colleagues
- Enthusiasm and a passion for delivering high-quality support services
- Ability to work independently and as part of a team, with a flexible and adaptable approach
- Current Australian Drivers Licence with no restrictions and a reliable, roadworthy motor vehicle for travel to customers' homes
