Social Worker, Cystic Fibrosis Unit Specialist

The Cystic Fibrosis Social Worker is a key member of the South Australian Adult Cystic Fibrosis Service team, responsible for maximizing the psycho-social adjustment of patients and their families.
Key Responsibilities- Attend and actively participate in multi-disciplinary clinical activities, such as ward rounds, outpatient clinics, and meetings.
- Undertake data collection and reporting as required.
- Provide assessment and interventions through consultation, counseling, liaison, and educational services within CALHN and in the community.
- Utilize organizational and community resources to achieve optimum outcomes for patients, families, and carers.
- Participate in quality improvement activities and research projects to support best practice and optimize service delivery and outcomes.
- Appropriate degree or equivalent qualification giving eligibility for full membership of the Australian Association of Social Workers.
- Current National Police Clearance (NPC).
- Working with Children Check.
- Immunisation screening as indicated in the role description.
