About the Role
We are seeking a dedicated Regional Care Coordination Specialist to join our team in Townsville. This is a Full-time role that offers a range of benefits, including up to $15,900 FBT salary packaging benefits and flexible working arrangements.
Job Description
The successful candidate will be responsible for providing exceptional support and coordination for our clients, working closely with them, their families, and healthcare professionals to ensure smooth transitions and the delivery of high-quality care.
Key Responsibilities
- Lead and collaborate with interdisciplinary teams to ensure the effective implementation of program objectives
- Provide guidance and supervision to program staff to ensure the highest standards of care and service delivery
- Develop and implement comprehensive therapeutic care plans for clients, considering their unique needs and circumstances
- Conduct consultations with community services to enhance collaboration and support networks for clients
- Maintain accurate and up-to-date documentation and clinical records in accordance with regulatory standards
- Facilitate smooth and well-coordinated discharge planning in collaboration with healthcare professionals, ensuring continuity of care for clients post-transition
- Provide support to clients and their families to address the emotional aspects of the transition period
- Deliver education sessions to clients and families, promoting health literacy and empowering them to actively participate in their care
- Offer ongoing support services to enhance the overall well-being of clients throughout the transition period
Requirements
To be considered for this rewarding role, you will have:
- Excellent communication and interpersonal skills
- Ability to work collaboratively with diverse stakeholders to achieve positive outcomes
- Previous experience in program leadership and case management
- Strong understanding of therapeutic care planning and discharge planning processes
- Ability to work collaboratively in a multidisciplinary team
- Effective prioritisation and time management skills
- Excellent written and verbal communication skills
Salary Information
The estimated annual salary for this position is around $90,000 - $110,000, depending on experience and qualifications.
