Job Summary:

This is a maternity leave relief position for a social worker in the Transitional Aged Care Program, covering 37 weeks with the possibility of an extension on a part-time basis.

The successful applicant will provide short-term therapy and support to aged clients on returning home from hospital, working closely with the program team to ensure seamless care.
