**Job Title:** Inpatient Care Coordinator

The primary responsibility of the Inpatient Care Coordinator is to provide strategic oversight and coordination of client care delivered within a multidisciplinary team. This role involves working collaboratively with clinical leads and other team leaders to ensure direction and guidance are provided to staff.

This position ensures compliance with models of care, business parameters, program reporting, and outcomes. The Inpatient Care Coordinator works closely with hospitals and referring providers to promote service integration, coordinated client care, and develop strategies to enhance client and community health.

Key responsibilities include:

  • Developing and implementing systems and processes for clinical governance and quality service provision;
  • Strategically overseeing monitoring and reporting on activity levels, resource allocation, program outputs, and review of strategies to ensure compliance with performance standards;
  • Reporting on financial variances applicable to each program managed;
  • Assisting in budget preparation and negotiation of contractual obligations with funding bodies.

The Inpatient Care Coordinator contributes a program perspective to strategic and business planning as a member of the leadership team.
