Physiotherapist - Home and Community Care Specialist

As a Physiotherapist in our Home and Community Care team, you will play a critical role in supporting older Australians to live independently at home and across our retirement living communities and residential aged care homes.
We are seeking a highly skilled and compassionate Physiotherapist to join our team, working closely with elderly clients in their own home to ensure their physical needs and goals are met.
Key responsibilities include:
- Providing in-home physiotherapy services to assist with mobility, pain management and strengthening
- Development of individual care plans to achieve personalised functional outcomes
- Falls prevention, education and strength training work
- Working closely with local community teams and key stakeholders
- Incorporating a holistic approach to healthcare that incorporates healthy living activities according to community and organisational priorities

To be successful in this role, you will need to be currently registered as a Physiotherapist with AHPRA, hold a Bachelor's degree in Physiotherapy or equivalent, and have prior community experience. You will also need to be able to provide high-quality treatment and intervention, hold a Police Check (dated within the last 3 years), and obtain annual flu vaccinations and evidence of Covid-19 vaccinations.
