We are looking for an experienced Registered Nurse to join our team as a Nursing Care Specialist. The successful candidate will be responsible for providing high-quality clinical care services to complex Home Care Package clients in their homes.

The Role:
  • To deliver high-quality clinical care services to Home Care Package clients in their homes.
  • To develop, implement, and review individualized care plans to meet client needs.
  • To conduct assessments to identify client requirements and ensure compliance with relevant standards.
  • To provide in-home clinical services, such as medication management, wound care, chronic disease management, and palliative care.
  • To act as a liaison between referral sources, families, and the care team to ensure seamless communication and care delivery.
  • To ensure compliance with all statutory and organisational policies.
Desired Skills and Qualifications:
  • A qualified Registered Nurse with current AHPRA registration.
  • Strong organisational skills and proactive problem-solving approach.
  • Excellent written and verbal communication skills and ability to build relationships and achieve positive outcomes.
  • Minimum 2 year's experience in a care management/coordination role within aged care or community care.
  • Proficiency in care management software and computer systems.
  • Current First Aid & CPR certificates and National Police Check.
