About Our Team

We are seeking a qualified Mental Health Clinical Nurse to join our multidisciplinary team. As part of this role, you will coordinate and deliver case management and care coordination activities, providing specialist assessment, crisis management, care coordination, and nursing interventions.

The successful candidate will have a caseload that supports individuals with serious mental illness, promoting recovery through advanced care planning, risk management, and the delivery of treatment and prevention activities.

Key Responsibilities:
  • Develop and implement case plans to support individuals with complex mental health needs
  • Collaborate with multidisciplinary teams to provide comprehensive care coordination services
  • Provide specialist assessment and crisis management services as required
Requirements:

To be successful in this role, you will need to possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

You will also require relevant clinical experience and knowledge of mental health principles and practices.
