
Primary Care Nurse Practitioner

We are seeking a compassionate and skilled healthcare professional to join our dynamic Primary Care team as a Nurse Practitioner in Community Rapid Response Service.
This is a part-time position working 48 hours per fortnight, based in North West Tasmania, with opportunities for professional growth and development.
- Provide rapid response nursing care to patients in community settings.
- Conduct comprehensive health assessments and develop treatment plans.
- Collaborate with multidisciplinary teams to ensure continuity of care.
- Support patients and families through acute health episodes.
The successful candidate will have current registration as a Nurse Practitioner with AHPRA, experience in acute care, emergency, or community health settings, and strong clinical assessment and decision-making skills.
Key Responsibilities- Rapid response nursing care in community settings.
