We are seeking a skilled Social Worker for a locum position in regional Victoria. The role involves working with a team of healthcare professionals, including other experienced Social Workers.


Job Description

This is an exciting opportunity to work on the Transitional Care Program / Rehab in the Home / GEM Program, providing community follow up support to patients and their families. As a key member of the multidisciplinary team, you will be responsible for assessing patient needs, developing care plans and coordinating services to ensure optimal outcomes.


Required Skills and Qualifications
  • Knowledge of Victorian and NSW Aged Care systems is highly desirable.
  • You must have excellent communication and interpersonal skills, with the ability to build strong relationships with patients, carers and healthcare professionals.
  • As a confident and self-motivated individual, you will thrive in a fast-paced environment and be able to manage competing priorities effectively.
