Care Coordinator

We are seeking an experienced nurse or allied health professional to join our Western Health at Home Care Coordination team in a part-time, temporary role until 18 May 2025.
About the RoleThe Care Coordinator will play a crucial role in providing clients and carers with care coordination and management of their chronic illness, and navigation of the Health and Primary care system. The successful candidate will be responsible for comprehensive patient assessments in the home, providing self-management support and coordinating patient healthcare needs across the healthcare continuum.
Key Responsibilities- Conduct thorough patient assessments in the home to identify care needs and develop comprehensive care plans.
- Provide self-management support to clients and carers to empower them to manage their chronic illness.
- Coordinate patient healthcare needs across the healthcare continuum, ensuring seamless transitions between healthcare services.
- Work collaboratively with healthcare professionals to ensure clients receive the right service mix to safely support care in the community.
- Appropriate qualification in nursing or eligibility for registration with relevant body.
- At least 5 years of experience in a community nursing setting with recent acute care experience.
- Experience in assessment and management of people with chronic disease and complex needs, discharge planning, and care planning.
- Clinical experience and knowledge within Mental Health Services is highly desirable.
