About the Role

As a Support Associate, you will be responsible for providing person-centered support to individuals with disabilities in their own homes or community settings. You will work closely with our clients and their families to understand their needs and preferences, and develop personalized support plans that promote their independence and well-being.

Key Responsibilities

• Assist clients with daily living activities such as personal care, meal preparation, and medication management.
• Support clients in participating in community activities and outings.
• Provide emotional support and companionship to clients.
• Collaborate with other healthcare professionals to ensure seamless delivery of care.

Requirements

• Aged care experience or similar field.
• Mental health support skills.
• Personal care assistance expertise.
