Clinical Care Coordinator Position

About the Role:

This is an exciting opportunity for a Clinical Care Coordinator to lead clinical and care teams, ensuring residents receive high-quality care while fostering an understanding of their unique identities and experiences.

Key Responsibilities:
  • Provide comprehensive clinical and specialized nursing care, documented and tailored to each resident's needs.
  • Prioritize residents' preferences, independence, dignity, and identified care-related risks in decision-making processes.
  • Oversee and conduct scheduled AN-ACC reappraisals, including mandatory and voluntary assessments, as required.
  • Hold Registered Nurses (RNs) and Enrolled Nurses (ENs) accountable for daily case reporting, care planning, and addressing patient concerns promptly.
  • Conduct routine clinical assessments to address identified gaps in care effectively.
Requirements:
  • Current registration as a Nurse with AHPRA, with no restrictions.
  • Senior management experience in the aged care sector.
  • In-depth knowledge of the aged care industry, with a keen eye for emerging trends and best practices.
  • Proficiency in managing human resource matters, complemented by strong negotiation skills.
  • Skills in utilizing various information technology tools and software, including Word and Excel, with adaptability to new skills.
