Role Overview:


We are seeking a dedicated Clinical Care Coordinator to join our team at Brightwater Care Group. As part of a multi-disciplinary team, you will be responsible for assessing, planning, implementing, and evaluating clinical care for clients.



About the Role:


This is a key role in ensuring that clients receive high-quality care and support in accordance with their individualised Care Plans. You will work closely with other healthcare professionals to deliver person-centred care and promote choice, independence, and quality of life for clients.



Main Responsibilities:



  • Clinical Assessment: Conduct thorough assessments of clients' needs and develop effective care plans in collaboration with multidisciplinary teams.
  • Care Delivery: Deliver inclusive and quality care services that actively promote client autonomy, dignity, and well-being.
  • Communication: Maintain open and clear communication with clients, families, and healthcare colleagues to ensure seamless care delivery.
  • Leadership: Lead shifts with empathy and mentorship, promoting a positive work environment and staff development.


Required Skills and Qualifications:



  • AHPRA registration as a Registered Nurse or Eligible Midwife
  • Demonstrated expertise in clinical care coordination and leadership
  • Excellent interpersonal skills and ability to communicate effectively with diverse stakeholders
  • Capacity to prioritise workload safely and efficiently, focusing on client needs
  • A satisfactory National Police Clearance (less than 1 year old)
  • Current First Aid Certificate (less than 3 years old)
  • Satisfactory NDIS Worker Screening check
  • Evidence of up-to-date influenza vaccination
