About the RoleWe are seeking an experienced Registered Nurse to join our team as a Healthcare Specialist - Community Nursing. In this role, you will have the opportunity to make a real difference in the lives of our clients by providing high-quality nursing care in their own homes.
The successful candidate will be responsible for:
- Providing health promotion and education to clients and carers;
- Promoting and delivering our award-winning Wellness for Independence programs in person and virtually;
- Monitoring and promoting health outcomes via Telehealth;
- Providing direct care services across various funding programs, including wound care, diabetes care, medication management, stoma care, and catheter care;
- Assessing, developing, and evaluating nursing care for all clients through a diverse and collaborative approach;
- Working with clients to develop and achieve client care plans that are goal-driven and client-centred;
- Assisting clients to become independent with managing health needs through a Wellness and Reablement approach to achieve levels of function in conjunction with Allied Health clinicians.
To be successful in this role, you will need:
- A Division One Registered Nurse qualification with the Australian Health Practitioners' Regulation Authority (AHPRA);
- Experience working as an independent clinical practitioner or in a team;
- Strong communication and interpersonal skills;
- Ability to work systematically and accurately;
- Proficiency in electronic technology, including smartphones, tablets, laptops, email, internet, and client data management systems;
- NDIS Worker Screening Check or willingness to obtain;
- Current driver's licence and ability to travel for client care.
