Nurse Care Coordinator

This role involves providing exceptional patient care in a Haematology Outpatient Unit.
Key Responsibilities:- Conduct comprehensive and accurate nursing assessments of patients to inform care planning.
- Identify and implement necessary discharge-planning requirements for seamless patient transitions.
- Collaborate with the multidisciplinary team to develop effective care plans that meet individual patient needs.
- Implement prioritized care strategies to achieve desired outcomes, while continuously evaluating and refining interventions as needed.
- Ensure timely, complete, and accurate documentation of nursing care and patient progress in medical records, facilitating clear communication among team members.
- Current Authority to Practice registered with the relevant professional body.
- Demonstrated recent clinical experience in Cancer Services Nursing is highly desirable.
- Ability to work independently, efficiently, and effectively during periods of high demand and activity.
- Proficiency in Chemotherapy administration and IV Cannulation (IVC) is essential.
- Excellent interpersonal and teamwork skills, with a proven ability to collaborate effectively within a multidisciplinary team environment.
