Care Coordinator Role

We are seeking an experienced and compassionate Care Coordinator to lead the delivery of high-quality clinical care for residents with complex needs.

In this pivotal leadership role, you will oversee resident assessments and develop tailored care plans. You will also coordinate clinical documentation and compliance reporting, as well as lead and mentor a team of Registered Nurses, care staff, and volunteers.

The successful candidate will have a strong background in aged care or acute clinical settings, with a minimum of 3 years' post-graduate experience. They will possess excellent leadership and workforce management skills, as well as strong clinical assessment and care planning capabilities.
