The Position
Position Title: Nurse Practitioner - Transition Care Program/Residential In-Reach
Classification: NO1 - NO2
Salary per hour (exc. super): $68.29
Hours per fortnight: Up to 76
Employment Status: Fixed Term Full or Part Time
Other requirements: COVID-19 Vaccination (3 doses)
This Regional Nurse Practitioner position is part of a project that aims to provide high-level clinical assessment support to Residential In Reach teams across the region.
The Nurse Practitioner will function autonomously and collaboratively in an advanced clinical role through telehealth consultations to provide high-level clinical assessment support. The Nurse Practitioner will report to the Manager of the RIR program and will be provided with clinical governance support and ongoing mentoring from a Geriatrician.
The NP will deliver telehealth support for Residential In Reach teams to support patient-centered care, operating within a nursing model of holistic practice and maintaining a focus on best patient outcomes.
