Job Summary

We are seeking a skilled and compassionate Home Care Allied Health Professional to join our team. As a Home Care Allied Health Professional, you will play a vital role in providing high-quality care and support to older adults in their own homes.

Key Responsibilities

  • Provide allied health services to older adults in their own homes, including dietetics, exercise physiology, occupational therapy, physiotherapy, podiatry, and social work.
  • Work collaboratively with our multidisciplinary team to develop and implement individualized care plans.
  • Conduct assessments and provide interventions to promote the health and well-being of older adults.
  • Communicate effectively with clients, families, and other healthcare professionals to ensure seamless care coordination.
