About the Role
We are seeking a highly skilled and compassionate Discharge Planner to join our team at Sunnybank Private Hospital. As a key member of our healthcare team, you will play a vital role in ensuring the smooth transition of patients from our hospital to their next stage of care.
Key Responsibilities
- Collaborate with multidisciplinary teams to coordinate discharge services, ensuring they are patient-centered and adhere to best practice.
- Develop and maintain relationships with community services, professional groups, and support organizations to provide ongoing care and support to patients.
- Identify and escalate potential issues or concerns affecting patient discharge, working closely with nursing and medical management teams to resolve them.
- Support patients and families in the discharge process, including referrals to residential aged care facilities.
- Contribute to quality improvement initiatives to enhance patient care and reduce readmissions.
