Job SummaryWe are seeking a skilled Care Coordinator to join our HomeCare team in Tea Gardens, NSW. As a key team member, you will play a vital role in delivering a variety of services to meet our clients' unique circumstances and needs.
Key Responsibilities- Develop and manage care plans for a portfolio of clients, ensuring their needs are met and exceeded.
- Oversee a range of administrative tasks, including drafting budgets, monitoring financial transactions, and actioning changes in care.
- Liaise with external providers and promote our services to ensure seamless delivery of care.
- Build and maintain interpersonal relationships with staff and clients, fostering a positive and supportive team environment.
- Demonstrated experience in HCP, CHSP, DVA, case management, assessment, care planning, and service coordination.
- Current CPR Certificate and a valid driver's license with own fully insured vehicle.
- Solid understanding of client-centred, community-based care service delivery and associated quality standards.
- Ability to work with and adhere to budgets, as well as create and maintain interpersonal relationships with staff.
