Job Title: Community Team Lead
Job Summary:
Catholic Healthcare is seeking a highly motivated and experienced Community Team Lead to join our Home and Community Services team. As a Community Team Lead, you will be responsible for leading and managing a team of Community Workers, providing direct care to our older clients across Wagga Wagga and Young.
Key Responsibilities:
- Team Leadership: Provide day-to-day supervision, direction, and management of Community Workers.
- Compliance Management: Manage compliance requirements for Community Workers within Home & Community Services policies and procedures.
- Mentorship: Mentor Community Workers by planning, directing, and providing feedback from competency assessments/support visits.
- Performance Management: Oversee the work of Community Workers, including conducting performance reviews, Performance Improvement Plans (PIP), and provide guidance and direction to Community Workers to model and improve performance.
- Collaboration: Work closely with Coordinators and Care Advisors, providing feedback and addressing any service delivery concerns as they arise.
- Regional Management: Work closely with the Regional Manager to ensure Community Worker numbers meet the service demands of the region.
- Incident Management: Investigate incidents and complaints relating to Community Worker performance.
- Leave Management: Planning and management of leave requests for Community Workers.
- Community Meetings: Organising, planning, and documenting community worker meetings.
- Employee Engagement: Ensure timely and effective resolution of concerns raised by Community Workers.
- Feedback and Recognition: Ensure Community Workers are provided with regular feedback, are engaged, and positive behavior is recognised.
About You:
We are looking for a diligent, empathetic, and driven individual who is motivated by their passion to assist others. The ideal candidate will have:
- Experience in Supervisory Position: Demonstrated experience in a supervisory position and some experience in managing performance.
- Communication Skills: High level of verbal and written communication skills.
- Rostering Management: Understanding of rostering management systems.
- Home Care Experience: Previous experience as a Home Care Worker (min 3 years) or equivalent is highly desirable.
- Certification: Cert lll or lV in Aged Care or equivalent preferred.
- WHS Knowledge: Demonstrated knowledge of WHS policy and processes.
- Computer Literacy: High degree of computer literacy with an ability to learn new software programs.
