Physiotherapy Specialist - Integrated Care Model

We are seeking a highly skilled and experienced Physiotherapist to join our team in an integrated care model. This role involves providing physiotherapy services to patients across various settings, including hospital, community, and home-based care.

The successful applicant will have a tertiary degree in Physiotherapy and be registered as a Physiotherapist with the Australian Health Practitioner Regulation Agency (AHPRA). They will also possess excellent communication and interpersonal skills, with the ability to work effectively with multidisciplinary teams.

  • Support safe discharge of patients from hospital and promote hospital avoidance strategies">
  • Develop and implement preventative health strategies and chronic disease management plans">

Required Skills and Qualifications:

The successful applicant must hold a tertiary degree in Physiotherapy from a recognised institution and be registered as a Physiotherapist with AHPRA. They will also require:
